According to a February, 2012, update from Cochlear’s CEO, Dr. Chris Roberts, the cause of the failures was determined to be “unexpected variations in the brazing process.” In an article dated October 16, 2012, Cochlear’s Chairman, Rick Holliday-Smith, was quoted as stating: “When you try to weld or glue titanium and ceramic, most people have no clue how to do it – it’s a very complex thing to do.” Chairman Holliday-Smith was further quoted in the same article: “When you change any sort of form, there’s always a risk.” However, the most disturbing quote attributed to Cochlear’s chairman in the October, 2012, article is: “The question is how much testing you have to do to satisfy yourself while staying commercial.”
